• Online Portal: For the fastest and most secure way to complete your paperwork, we provide a paperless HIPAA compliant portal. Signing your forms has never been easier! You can request access by calling or texting us at (941) 867-0047, or you can email [email protected] for assistance. 
• In-Office: If you choose to wait until your appointment, please arrive at least 15 minutes early to allow proper time to complete the forms. At this time, you can complete the forms on the portal from your phone or a tablet provided in the office.
